• All IV and practitioner led services require medical clearance and practitioner approval.
• Membership benefits are non-transferable.
• Membership discounts may not be combined with other promotions.
• Cancellation: 30 days' written notice required. No cancellation fees.

• Members must be 18 years of age or older.
• The $99 rate is locked for 2 years from the date of enrollment.
